Use files from 2019 repo, update form according to newer specs:
- User can add multiple contributions + contributors for one musical work
- User can pick the ordering of their section (i.e. the first section defaults to Kyrie but the user can specify)
- Genre (style & type) and location autofills from database (but new styles, types, and locations may still be inputted)
- General css improvements, particularly making spacing regular
- Eventually will have autocomplete form VIAF and Wikidata (this is marked in another issue)
