Plan exploration still defaults grok

This PR moves parallel codebase exploration to gpt-5.6-sol-max via the how explorer role and the /setup-pstack template, but plan.md still tells multi-phase planning to pass grok-4.5-fast-xhigh for code explorers. Planning and how flows can spawn different models for the same exploration work.

Removed the Opus and GPT-5.5 default slugs. Panels now default to fable / sol / grok, with judgment on fable and specified code on sol.
